HOSPITAL APPOINTMENTS
From the moment you arrive, our team is focused on making your experience smooth and stress-free. You’ll be welcomed by our front office team, asked to complete or submit your forms, and then guided through your appointment. Dr. Peloza and our clinical staff will spend time understanding your symptoms, reviewing any prior imaging or medical records, and performing a detailed physical examination.
To help get the most out of your visit, please bring the following with you to your appointment:
Photo ID and insurance card
Recent medical imaging (MRI, CT, X-Ray)
List of current medications
Prior treatment history or surgical records
Referral information, if required by insurance
If you don’t have access to your imaging, we may be able to request it on your behalf, please let us know before your appointment.
